Overview

HHI provides the opportunity for a student to continue his/her current educational program as far as practical and assist him/her in maintaining school credit and achievement at the home school.

Credentialed teachers provide Home Hospital Instruction for 180 to 300 minutes per week.

Eligible students are those who incur a temporary disability that makes attendance in regular day classes or alternative education program impossible or inadvisable.

Only students expected to miss at least two weeks of school will be considered for HHI instruction services.

HHI is a service, not a school; all students need to be enrolled in a CUSD school to received HHI. For long term disabilities contact the Coordinator, as each student's needs are unique. (AB 1689)

Eligibility

Students who incur a temporary disability that make attendance in regular day classes impossible. A "temporary" disability is defined as a verified, by a physician or mental health professional, of a short duration. (2-6 weeks) The student is then expected to return to the same school and classes.

  • A temporary disability does not include a disability for which the student has been identified as an individual with special needs and is eligible to receive an IEP.
  • A student who is admitted to a hospital or health care facility within CUSD boundaries is eligible.
  • A student who is diagnosed by physician as pregnant AND unable to attend school on a regular basis is eligible.
  • When a student is on the HHI Program, he/she should be home between 8 a.m. and 3 p.m. can not be employed, and the home school must approve co-curricular activities.

Parent/Guardian Responsibilities

Getting Started

  • Notify the school principal, counselor or GIS that your child is under the care of a physician and will be unable to attend school for 2 or more weeks.
  • Obtain verification from physician. Print the Request and Application form (HSP 1,2,3) to take to the physician.
  • Present the signed Request and Application form (HSP 1,2,3) to the Counselor or GIS at your child's home school.
  • Obtain homework for the your child until you are notified that a HHI Instructor has been assigned.

During

  • Arrange with HHI teacher appointments to meet with the student. An adult must be present.
  • Have a suitable, well-lit and quiet place for the instructor to work with the student.
  • Notify the instructor in advance if there is a reason the student is unavailable for the appointment.

After

  • Notify the instructor when the student is ready to return to school.
  • Complete the HHI Parent Survey Form.


Home School Responsibilities

  • When the parent/guardian notify the school that the student has a temporary disability requiring an extended absence from school (two weeks or more) the administrator completes a Request and Application form (HSP 1,2.3).
  • Assist the HHI instructor by requesting assignments from the student's classroom teachers.
  • HHI Coordinator will obtain Home Information and Schedule from the district database.

Classroom Teacher Responsibilities

  • A student assigned to HHI short term is still under the jurisdiction of the classroom teacher.
  • The teacher retains the responsibility for providing assignments, tests, and the authority for issuing grades.
  • When a student becomes long term after 6 weeks, the HHI instructor may continue the work with the classroom teacher to modify the student's assignments.
  • If the student is unable to complete all course work, the classroom teacher has the discretion to give an Incomplete (I) grade within the guidelines of each school's policy.

Home Hospital Instructor Responsibilities

  • The instructor contacts the parent and arranges an appointment or schedule.
  • Briefs the parent/guardians on their policies, procedures and responsibilities.
  • Contacts school for assignments, tests, and materials.
