  • CalPERS Special Power of Attorney

    A CalPERS special power of attorney allows you to designate a representative or agent, known as your attorney-in-fact, to conduct your retirement affairs. Should you become unable to act on your own behalf, your designated attorney-in-fact will be able to perform important duties concerning your CalPERS business.

  • Signature for 403(b) or 457(b) Transaction Form

    If you need a signature on a document for your 403(b)/457(b) due to cashing out, rolling funds, etc.  CUSD is not authorized to sign documents.  OMNI, the districts Third Party Administrator, can sign those types of documents. Please call OMNI at 877-544-6664 and they can give you direction.
